Policy No.: 002
Revision No.: 2
Effective Date: 3/31/97
Category: General Information
Title: Policy Formulation

Any interested University employee or faculty member may recommend what he/she considers to be an essential or required policy and subsequent procedure for its implementation. Initially, the proposed policy should be submitted to the appropriate Department Head or Chairperson for review and formalization. If the proposed policy is then considered to have applicability, it would be recommended formally to the Director of Business Affairs & Auxiliary Services for consideration, analysis, and approval.

If approval for implementation is obtained, the completed policy and procedure would then be distributed appropriately by the Director of Business Affairs & Auxiliary Services.

The policy categories as listed below are intended to describe general departmental areas of applicability. A numerical coding system identifies the topical categories and specific polices related to each. The system utilizes the 000 category for General Information with listings 001, 002, and so on. The system should permit expansion to continue sufficient to University needs.

000 - General Information
100 - Business Affairs & Auxiliary Services
200 - Controllers Office
300 - Computing Services
400 - Human Resources
500 - Physical Plant
600 - Public Safety & Security

A decimal system is used to insert specific policies restricted to each category. For example, a policy specifically related to the "100" category would be numbered 100.1. This permits indefinite expansion of the number of policies related to each category.

If the policy is revised, the number of the revision will be noted. It is understood that a revision will supersede any previously existing policy.

All questions related to the formulation, maintenance, and distribution of University Policies and Procedures should be directed to the Director of Business Affairs & Auxiliary Services, extension 9002, third floor, Presidents Hall.
