Instructions for form use. Refund Request. Refund Request Procedures. Forms must be opened using Adobe Acrobat.  Use the button Get Adobe Reader to download the software if needed. How to use the on-line forms: Save a copy the blank document to your local drive for your own records. Place your cursor at the beginning of the space where you wish to begin typing. Fill in all of the blanks that are required. Print out the completed form by clicking on the printer icon within Acrobat reader. Fax, mail, or take the completed form to Enrollment Services.
